South Eastern African American Center of Excellence in the Elimination of Disparities in Diabetes

The purpose of REACH US: SEA-CEED is to eliminate health disparities related to diabetes prevention and control and to prevent and reduce risks and complications related to hypertension, stroke and amputations in African Americans at risk or with diabetes in Alabama, the District of Columbia, Florida, Georgia, Maryland, Mississippi, North Carolina, South Carolina, Tennessee and Virginia with African American populations greater than 20%. The coalition is focusing on policy and systems and the related behavioral health areas of physical activity and nutrition.

The SEA-CEED community action plan addresses geographic areas with high disparities, builds on the success of previous REACH coalitions, capitalizes on a broad network of academic and community partners, applies community based participatory action principles and evidence based guidelines, and works at the local, state, and multi-state levels, and with CDC.
